The next thing I'd do is analyze one case closely where I think it should have charged a fine:
* Itemtype, home and holding branch of the item * Patron cateory and home branch of he user who has checked the item out * CircControl setting * finesmode setting * MaxFine setting * Calendar: is today a holiday?
