Hi
When trying to configure an icon for an item type, the chosen icon won't
show in the Koha menu "Administration > Item types". I used to see the
icon in the first column entitled "Image" - now this column is empty.
Also the icon won't show in staff interface or OPAC.
As I can see while saving the page

slow, with command "top" I see the following:

I tested this on a newly installed Koha 16.05.05.
I also tested it on my own Koha demo installation (16.05.00)

I also tested it on the Catalyst demo installation (16.05.04).

It's everywhere the same - the icons won't show when they are chosen
fpor a certain item type. Is this a bug or do I miss something essential?
