I've started rolling out mod_pagespeed ( https://developers.google.com/speed/pagespeed/mod) on our apache servers to prevent just this problem. It'll re-write the css files with a unique hash in them based on their contents, so when they change, the filename sent to the browser also changes, forcing them to refresh. Just a nice little tip.
