Item type may or may not make sense to the user depending on how the library uses it. In my library item type would make no sense, but collection code would. I think this latest version of the patch works well, but I agree that label should change according to the selection. We shouldn't presume that the correct term ("item type," etc.) won't be understandable to the patron. Better to let the library decide to customize it somehow.
