Currently enrollement and hold fees are setup at membership category level.
This is not sufficient for a multi library setup where different libraries have
different fees for each category.
In addition to enrollment and hold fees, this change will include two
additional fees: renewal fee and duplicate card fee.
In the case of renewals there will also be a grace priod. If memmbership is
renewaed within the grace period, a discounted renewal fee will apply.
