USB stops responding on XPS9320

Bug description:
An intermittent problem, but to reproduce:
* unplug a USB device
* plug it back in
* notice it hasn't been detected
After this point, no amount of plugging or unplugging seems to change
anything: nothing appears in `dmesg`, `lsusb` shows only internal USB
hardware, and video-over-USB devices do not appear in
`/sys/class/drm`. Both USB ports are similarly unresponsive. The only
remedy appears to be to reboot.
`dmesg` shows the unplug event, but nothing after that:
