xhci lockup

Bug description:
when I'm playing music through a Blue Yeti audio device, the audio
occasionally dies at PulseAudio (device disconnect) and then exactly
10 seconds later all of xHCI restarts and refuses to come back up.
this happens sporadically about every 2-3 hours if the audio device is
in use. I tried disabling usb autosuspend with no effect. the system
otherwise works fine even when usb subsystem is dead (can ssh in etc)
