This failure occurred after replacing GRUB with systemd-boot, which required (a) mounting the ESP to a new directory, /efi; and (b) replacing /boot with a symlink to /efi. Other instances of this configuration but without the symlink failed to update the kernel, witn `uname -r` returning a prior version.
