ISP2532-based 8Gb Fibre Channel to PCI Express HBA (rev 02) on ubuntu 14.04 LTS and 16.04 LTS. Problem seems to be similar to https://bugs.launchpad.net/ubuntu/+source/linux-firmware/+bug/1117428 but I already have newest kernel and it happens on two version of ubuntu.
