Can confirm! Perfectly describes my issue too. Has been happening since I
updated my KDE installment to 5.27.1 (latest as of this comment). For
reference, I tried changing it off ~./.config/xdg-desktop-portal-kderc and
putting in the string "Show Inline Previews=true", yet no effect.
Operating System: Arch Linux (EndeavourOS)
KDE Plasma Version: 5.27.1
KDE Frameworks Version: 5.103.0
Qt Version: 5.15.8

Unfortunately, not tech-savvy enough to understand these terms, but if this is
any help, it's the file dialog off any program that calls off the KDE file
chooser when you want to upload into or save a image off, like Firefox.
Interestingly enough, this bug seems to have been resurfaced. First discovered
on 2021, thanks to this user off KDE forums:
