They are working, they're just confusing as hell; See Bug 433841. We should
probably replace them with more standard style shortcuts.

What you need to do is keep your finger on the modifier key and press both
letters, one after the other. For example to trigger the action with the
shortcut "Alt+D, Alt+A", you put your thumb on the Alt key, then press the D
key, then--without taking your finger off the Alt key--press A.

For the one with the shortcut "Alt+D, A" you press Alt+D, then remove your
finger from the Alt key, then press A.

Yes, it's ridiculous, I know. :)
