SUMMARY
When I want to add a new radio station to Elisa I end up with a form which is
empty except for 4 labels: title, http stream, additional info and image.
There are also the 3 buttons: Change, Apply and Close of which only Close is
enabled
STEPS TO REPRODUCE
1. open Elisa
2. select Radio's on the left
3. press + button in the middle to add a radio
OBSERVED RESULT
Just a gray form, no input boxes
EXPECTED RESULT
Input boxes to add the radio stream info

Linux: Tumbleweed
