Firstly can you run
- running "kcmshell5 qtquicksettings" and setting the render loop to basic
and report if that makes a difference and report back.
Secondly:

Running kde from startx is not supported, as I have wasted too much time in the
past dealing with what have turned out to be user setup issues.
Please reopen whilst launching from sddm
- output of env
- output of xrandr -q
- ideally a snapshot of gdb of what plasmashell is doing.
