There's different mute states.

You can have several audio devices (e.g. built-in speakers, bluetooth
headphones, bluetooth speakers, HDMI audio, and a USB sound card) at the same
time.

When you mute something in the popup, you mute that specific device, and all
other devices are unaffected (e.g. if you mute the speakers, then connect your
bluetooth headphones, those will not be muted just because the device was).

On the other hand, if you middle-click the widget, it forces a global mute -
the individual devices are left as they are, but the whole system is muted.
That's the "no sound period" option. It's state is not directly visible in the
popup, but it is visible in the right-click menu and the hamburger menu.

This is clearly somewhat confusing if you're not used to it, but it's not easy
to change because a lot of people also depend on this. There's several bug
reports and feature requests that go in a similar direction, like bug 511276
(basically the same for inputs instead of outputs) , and bug 435199. We
currently do not have a good way to solve these conflicting requirements and
edge cases.

I'm leaving this bug open separately for now as I couldn't find one talking
explicitly about outputs, but we might merge it with bug 511276 later as we'll
likely do the same thing for inputs and outputs - otherwise it would be even
more confusing.
