duplicated menu item
DESCRIPTION
With a generator that exports a format different than Plain Text, back swapping
the file via save doesn't clear the old export menu item. Clicking on the stale
item crashes Okular
STEPS TO REPRODUCE
1. Use a generator that exports to a format different than Plain Text e.g.,
okular-mupdf-ng
2. Add an annotation to the opening PDF, then save.
3. Open the File -> Export As menu, then select the 2nd duplicated item.
OBSERVED RESULT
Okular crashes
EXPECTED RESULT
Okular never crashes, the export menu has no duplicated items.
