Okay, I think I can reliably replicate at least one way that this "on what
monitor is the mouse" goes wrong.

In case it matters, I have the ALT-TAB task switcher "Flip switch" and I've
configured it to show only the windows that are on the screen where the mouse
is.

Whenever I ALT-TAB, I'll see the list of windows on the screen where my mouse
is, the other screens do nothing. So far so good.  If I keep ALT pressed (not
tab) the list of windows stays there but doesn't move.  

If I THEN move the mouse to another screen, then do NOT move the mouse, let go
of ALT, then press ALT-TAB again, I will see the task switcher ON THE WRONG
monitor, and repeated ALT-TAB presses continue to be on the wrong monitor until
I move the mouse a little, then suddenly the next ALT-TAB will be on the
correct monitor
