I have exactly the same issue, and I'm able to replicate it easily. 

Okular Version 25.12.2

1) Using Dolphin, open a PDF file with some form fields, hosted on a SMB share. 
2) Click on "Show Forms" to make them editable
3) Edit some text on any field and Save - WORKS
4) Edit some more text on any field (doesn't matter) and Save again - ERROR
(with the same message as other users "File could not be saved in
'/tmp/okular.xxxxx'. Try to save it to another location"

So, basically, the first save works, the second save fails and all next
attempts equally fail. Each time creates a new tmp file with a different suffix
and they all remain in the /tmp even after closing Okular. Permissions look
fine (both on /tmp and the files themselves).

If I copy that exact same file and copy it to local directory (for example
~/Downloads), I can save edit it and save it as many times as I want - no
errors ever).
So, at least in my case, the bug seems related to the file being open from a
network mounted directory - and even them, the first save works. So something
changes after that first save that triggers the bug.

This is 100% reproducible.
