Hi

Funny thing is that it is difficult to reproduce, probably the exact moment 
when i/o fails is deciding. I have collected following errors so far:

Unexpected error while saving file: Untitled.ipynb database disk image is 
malformed
The history saving thread hit an unexpected error (OperationalError('disk 
I/O error',)).History will not be written to the database.

I will try to debug precisely when login to jupyterhub will be not possible 
for some user.
