I have a form that works perfectly until I try to upload a file, please check out:
http://freshbump.scheetzdesigns.com/ Click the big blue "Add the Next Image" button and play with the form. If you don't add a file to upload, it works. Once you add a file, it's a no-go. There is documentation on how to handle the uploads, but the page gives no samples of code to actually use.
