I have this problem after I independently bring up the VM to modify it outside 
of Jenkins using the vsphere client.  Jenkins does not recover from this 
scenario nicely.  I have to launch slave agent manually on the node page after 
this and then everything works properly.

Updated jenkins to 1.648
Uninstalled vshere plugin, and installed it again (still have two entries in 
"new node").
Now the VM get cloned and appear in vshpere, but it keep offline state.
What should be done to get it online??
