Include built-in migration in 2.319.1 upgrade guide
Avoid duplicating content with the asciidoc include directive.
Split the existing content into a separate file for inclusion without
the documentation header at the top so that it can be reused in the
upgrade guide.
Add "Node Label Parameter plugin" to the list of plugins with known
compatibility issues.

Revert "Include built-in node migration in table of contents"
Daniel Beck noted that it was deliberately unlinked because the content
is only relevant to those who have seen the message from the
administrative monitor.
