Ah yes. Now it's all coming back to me. Developer tools, legacy. We did have a 
lot of templates, too; I had to check each one of those forms and make sure 
they were properly accessible.

If all you need is to insert form fields for data whose value can only be true 
or false, it is not necessary to complicate your life with macros/VBA.
A checkbox form field should be enough.
Regarding the fact JM described you about form creation not being the same on 
newer versions of Word, it effectively reffers mostly to an accessibility 
consideration. In Word 2007 and later, Microsoft implemented a new thing called 
“content controls,” which have existing functionality of classic form fields 
but also include capabilities such as deletion when completed which make them 
suitable for templates and other content reutilization scenarios.
The problem, as the Freedom Scientific tutorial for creating accessible forms 
in Word, is that those new content controls are not currently accessible with 
the keyboard. Fortunately Word still has the option, in the Developer tab 
together with the new content controls, a split button called “Legacy Control” 
which allows you to insert a form field like the ones available in Word 2003 
and earlier, which are backward compatible and more accessible with the 
keyboard and JAWS.
