   ## What
   
   Adds `docs/clients/codex.md`, a setup guide for the OpenAI Codex CLI, and 
links it from the README, the client index and the tutorial. Until now none of 
the client docs mentioned Codex.
   
   The guide covers all four ways to run the server:

   | HTTP | start with `PROFILES=http`, then `--url` | start with `docker run

   Each has the CLI form and the equivalent `~/.codex/config.toml` table. It 
also covers secured HTTP with `--bearer-token-env-var`, since the server never 
answers an anonymous `/mcp` request with 401 and Codex will not start an OAuth 
login by itself.
   
   ## Why
   
   Codex is a common MCP client and users had to work out the syntax 
themselves. The commands and TOML were generated by running `codex mcp add` 
(codex-cli 0.160.0) against a scratch `CODEX_HOME` and checking the resulting 
`config.toml`.
   
   ## Testing
   
   `./gradlew build` passes. The change is documentation only.
