Summary: Standardize the usage of the term IO in the documentation

The documentation refers to the concept of Input-Output in several places. The
references use the abbreviation of either I/O or IO.
Search the manual of styles (Chicago, Garner's, AP) for any guidance.
Even if no guidance is available, update the documentation to consistently use
any one form and ban the other.
