Description: In some cluster, we noticed Containers transitioned to closed
with proper consensus. Leader has more transactions, but the other two
followers agreed on consensus with lower transactions on applyTransaction
failures. This can lead to container state inconsistencies. SCM should have
stricter mechanism to transition the container to closer state. (was: In some
