Thanks, lots of test cases there :-). I see the one related to a case
insensitive filename, but you provided a patch for that.
It makes sense to continue addressing those ones that can occur in the
practical situations so your expert advice will be welcomed. If you see
ContentDisposition blocking some valid filename or other parameters in your
work then let me know please.
