WS-RM related faults do not contain the correct fault action in their
addressing action property.
According to the WS-RM 1.0 and 1.1 specs, the generic fault action URI to be
used is:
WS-RM 1.0: WSA_NS + "/fault"
WS-RM 1.1: WSRM_NS + "/fault"
