Have you checked that the credentials really reach the server. Could you try
this:
BindingProvider bp = (BindingProvider)serviceProxy;
bp.getRequestContext().put(BindingProvider.USERNAME_PROPERTY, username);
bp.getRequestContext().put(BindingProvider.PASSWORD_PROPERTY, password);
These lines should be inserted before calling the service.
