I don't understand the purpose of the #define constants SAPI_HEADER_ADD (not the enum value with the same name), SAPI_HEADER_DELETE_ALL and SAPI_HEADER_SEND_NOW. The two latter appears not to be used at all.
I don't understand the meaning of the SAPI_HEADER_ADD constant. I would assume that xxx_sapi_header_handler should just return a bool indicating whether the operation was successful or not?
