[Intel-gfx] Resend of the execbuffer/relocation improvements

Since everyone ignored the earlier patches I'm having to resend them to
attract attention. The first 10 are of immediate benefit and I've been
using them since the issue was raised at our f2f. The eleventh is really
laying the groundwork for ppgtt.
