Can't reproduce. Really sounds like an OS specific issue, if it is
affecting ALL browsers on a single OS/machine.
By the way, if I add the attachment with IE 11, the "open file"
dialogue shows up again immediately after adding the file. No idea
what that is.
This sounds like an issue (on Chrome) that was fixed a year ago.
Can't reproduce.
