If IMP will be the exclusive means of accessing the mail server, or you are not concerned about this behavior, this setting should be set to true.

But for message status, delete, new etc. it's working fine here with Cyrus as IMAP server and parallel access with Thunderbird (IMAP) and IMP/Horde. You have to wait for refresh for sure but this is standard IMAP behavior.

Or are we just bitten by a fundamental bug in the system?

Jan Schneider recently mentioned in another thread:
And you might try Horde 3.3.9-cvs which has a cache bug fixed.

I have downloaded a daily Horde snapshot and found in the CHANGES file:
Fix bug in Horde_Cache that would prevent cache entries from being
expired in certain cases, and prevent cache key collisions.

Could this have to do something with my observations?

Not sure maybe you should explain in more detail what is failing and what your test-cases are.
