anything you updated on either the server or monitored hosts at that time?

What happens if you run the NRPE check manually from the command line on the 
Icinga server (as the Icinga user) - what result do you get back from the 
remote host?

When you say "our 5xx servers", do you mean that you have between 500 and 600 
servers being monitored, or something else?

Could you get your custom check to write something to a log file on the remote 
machine being monitored, to record what parameters it received?
