I've seen so many of these and I understand why these conversions fail. The 
Colorado Child Welfare System springs to mind.

Have watched clients struggle with conversions while trying to bite my tongue. 
Some of the stuff is so obvious.

I can only ascribe it to the toxic table-banging management style that has 
become endemic in American business since the
1970's when the MBA degree was invented and those who manage no longer have to 
come up through the ranks (or marry
the boss's daughter).

Wish I could try my hand at leading such a conversion.
