I remember, when I was in Atlanta pre-IBM, we had our first UPS system involved. The electricians got everything installed and hooked up and decided to test the UPS.

It was the middle of the day.... Work day.... They cut the power without asking anyone. The entire computer area, computer room, production control and keypunch area when perfectly dark. It was in the basement. Fortunately the emergency lights came on.

It came to pass that someone had not read the instructions and did not activate the switch inside the UPS that enabled the batteries to supply power. It was still bypassed.

Oops......

When working for a third party disk vendor I was in a computer room and there was an IBM CE working on a 360/45. At one point I looked around and saw a column of smoke rising from the center of the processor. I called out to the CE and pointing, said, "Is that normal?"

He flew over a table and hit the EPO on the front of it. He then turned to me and said, "no, it isn't."
