I didn’t cause it, but I had to help fix it. At a prior employer, facilities staff were on the raised floor doing preventative maintenance on the chiller/air handler. While doing the PM, a big clump of dust fell onto the heater in the unit and started to smoke. Neither the facilities person, nor the operations staff could get to the halon dump cancel button in time. All power was cut from the room, and full halon dump ensued. This was at an office furniture manufacturer, so production lines on the factory floor soon came to a halt as well.
