The last time I experienced this it was because the VTOCIX dataset was
corrupted. I think I fixed it by running ICKDSF with
BUILDIX DDNAME(ddname) OSVTOC NOPURGE
followed by
BUILDIX DDNAME(ddname) IXVTOC
