Not business production critical, but...back at the community college where I was a student and eventually worked as lab manager, we had a few EPO buttons scattered around the public terminal room. One of these was next to a professor's office. One day near the end of the semester, a gentlemen was having some sort of issue with a program and was talking with said professor outside his door. Within a week, all EPO buttons in the lab had covers.

Luckily, during my lifetime, I've managed to avoid Halon dumps and bizarre EPO activations. However, I have had my share of UPS tests (both planned and unplanned) that didn't do what they were supposed to do. Heck, I've even had it happen once on a new UPS in my home data center.
