I am of course familiar with production-control schemes. Production must be orderly, but in my experience bureaucratic controls alone do not reduce errors: They only diffuse responsibility.

I have, on admittedly rare ocaisions, seen some of those controls catch errors, usually where programmers made invalid assumptions about input data quality and/or quantity.
