The first APAR on IEFBR14 of which I am aware occurred in the late 1960s in the very early days of OS/360. The error was that the program was not setting the return code to zero. I do not remember the exact date of the APAR, its fix, release number, etc. I would bet that Shmuel Metz does, though. :-)
