I get the same problem and I believe it is because the Heroku/Eclipse 
plugin is not persisting my Heroku Account Email and Password. Check your 
Preferences | Heroku and see if you have same issue.

Not sure if this is the plugin or Eclipse.

Sometimes it seems Eclipse has issues persisting security credentials and a 
fix is to create a new Secure Storage password - but this is not working 
for me on this occasion.
*Preferences | General | Security | Secure Storage* - Password pane and 
simply clicking Windows Integration and choosing Change Password

I am using Eclipse Kepler Service Release 2.
