Usually when this happens to me, I've pooched something so badly/ subtly that Mongrel is having trouble starting up. Sometimes this is a bug in my environment file or something. I would first look in your logs to see if you can find a trace of where the problem originates and then if that doesn't work I would suggest looking at any changes you recently made to routing, environments config, etc. for introduce typos or bugs and pay attention to the syntax check feature to make sure it's reporting that the files are okay inside Heroku.
