I'd use a large machine needle, probably larger than if I were actually machine sewing the fabric.  Holes in wool disappear fast when steam pressed, so I would not worry about any excess in size becoming permanent.  If the holes closed up before that, and especially if I wanted to do all the hole punching for a garment at once, I would use actual thread, perhaps something heavy like topstitching thread. Then sew without fastening off, and pull the thread out right before hand sewing.

I know there are knitting machines but are there any similar devices for crochet, suitable for home use?

Can you machine sew heavier fabrics with no thread to place the holes,
and then use the holes for hand sewing instead of having to pierce the
holes with your needle?

Hi,
I have a bit of the same problem. I've had to reduce the amount of sewing,
plan better (start months earlier), sew in shorter bursts, use longer
needles (cotton darning needles preferably because of length and long
head), sometimes using some kind of supporting bandage on my wrist. I also
try to avoid wool fabrics that require linen thread and force when sewing.
Softer wool sewn with wool thread and linen sewn with thinner thread than
I've previously used, also eases the pain. Silk is a pain no matter what. I
sew across my lap from right to left which keeps my wrist more stationary.

I can't sew like I used to and I've had to learn new ways of knitting
(combining different styles from all over the world, youtube can be your
friend), but at least I can still sew and knit. Crocheting kills my wrists
and hands still, so years pass between each time. I have to forget the pain
first :-D
