My reading of that thread is that doing the adjustments automatically requires precise knowledge of the fonts involved - something that would be even harder to do in Org (Org does automatic org markup -> groff markup conversion). So, it looks like these fine details are out of interest for ox-man.el library. I see no good way to do it automatically.
