The metrics for `ah' are OK -- this is a spacing glyph.  If you look
into old AFM files (which still have the `CC' command to build
composite glyphs) you can see that the caron is positioned
individually for each composite.

Just a fun fact.
TODO from groff 1.02 has the line:

 Per font composite character mechanism

That was year 1991. A long term project, it seems :(
