It wouldn't be too difficult to separate the PDF into pages (with added barcodes) if your spec could be modified to return pdf pages instead of images of them.
Otherwise you would need to have an-off-appengine process that converted PDFs to images, unless you can find/write a PDF renderer in python/java.
