Did you source your .bashrc after adding the environment variables?
Opening a new terminal window and running configure from there should
work, too.
Alternatively, set the appropriate environment on the fly, like so:
CPPFLAGS=-I/home/cm/install/fftw-3.2.1/include LDFLAGS=-L/home/cm/
install/fftw-3.2.1/lib ./configure --enable-float --prefix=/home/cm/
install --program-suffix=_h
