Can't speak for William, but it was a case where using older/early versions of btrfs tools from some live-USB you found at the bottom of your bin of spares could cause worse damage and data loss on btrfs. I recall the devs recommending to always use the latest version if you were attempting a recovery of a damaged fs and seek advice if in doubt.
