You could look at lifewithqmail.org or google for "qmail toaster". We are
using a custom build with almost everything hooked into MySQL databases
(even WebMail preferences, address books, spamassassin options, etc).

The only maintenance is adding/removing/modifying account - we have
automated everything else across our MXes.

I know practically nothing about mdaemon - what format are the mailboxes?

We migrated over a year ago from sendmail -> qmail. I ended up writing a
lot of perl scripts to make the transition as painless as possible. There
are scripts out there for converting from mbox to maildir formats.

The "usual" clamav + spamassassin combo.
