Hey Even,
I tried using gdal2.0.2 (deployed with QGIS 3.0) and got this result.
The original Tiff is converted to level 17 of GPKG, and then after the gdaladdo
I have level 18 through 10, where 10 has 2 tiles.
This is the output of gdalinfo:
